Time for Endo, Internal bleaching, and Restoration

A patient seek for the cure to a dark tooth which causes a restricted smile. This tooth had filled with composite resin, and the patient had experience of spontaneous pain regularly. Xray examination showed an extensive radiopaque (filling material) onto the pulp chamber, vitality test was negative, and slight tenderness on percussion on upper left second incisive.
After taking proper informed consent, i started to dig up old composite fillings. I still had a hope that this tooth still vital, but when i finished digged up all the filling, i found out the pulp chamber already opened, the pulp was already shrink or gone, and had a trace of previous devitalization process done by previous operator..

Cleaning and shaping was done with Silk Files (Many) and Protaper Universal (Dentsply), plus Natrium Hypochlorite 5,24% as the irrigation.. right after coronally sealed orifice with low_shrinkage_stress flowable composite (filtek_bulkfill flow from 3M), the cavity was internally bleach with 35% hydrogen peroxide from YouTuel..
A week after, i removed all the previous fillings, and place a new composite resin material (Palfique LX5 from Tokuyama), polished it with Soflex (3M) and Enhance (dentspy). Xray evaluation with VistaRay 7.1 (Durr) showed a homogeneous restoration body and intact margin on this tooth.
